The address is 2750 11th Street in Rock Island. ·         Project made possible by funds from Affordable Care Act·         12 adult exam rooms\, 10 pediatric exam rooms\, 8 dental operatories\, 2900 sq. ft. meeting room·         New building has doubled the size of the previous dental clinic – from 4 operatories to 8·         Founded in 1975\, Community Health Care offers a medical and dental home for all those in need in the Quad Cities area.  It also acts as a safety net for people who\, for financial\, social or cultural reasons cannot access primary health care elsewhere in the community.  Community Health Care offers preventative\, acute and chronic care\, as well as laboratory\, x-ray\, pharmacy\, nutrition\, and health education services.
