Nurse Care Coordinator-Iowa RN License

Position Details

Family Resources is looking for a strong, determined, passionate, innovative, and trauma-informed Nurse Care Coordinator. The Nurse Care Coordinator interviews and guides clients and their families with behavioral/mental health conditions as a member of the Comprehensive Care Coordination Team. Provide family-driven and community-based, comprehensive, and culturally competent care in the provision of services. Provide health education to individual members and families and develop health education programs for population health management. Responsible for IHH care coordination of FRI residential clients. Provides nursing support to Foster Group Care clients. Provides nursing consultation as needed by other FRI programs. 

  • Position Hours: Monday-Friday Approx. 8am-5pm, flexible as needed – some remote work

Essential Duties and Responsibilities

  • Provide planning, care, and coordination functions, including follow-up contacts with family and community partners.
  • Coordinate and triage CCC referrals, working with referral sources and provider agencies, providing consultative services.
  • Interview clients and their caregivers to obtain and interpret social history information and/or problems and barriers.
  • Provide health education, monitoring, and support, with follow up to children and their families, as well as community partners.
  • Develop and implement multidisciplinary care plans, based on the principles of family-driven care.
  • Evaluate and factor in the client’s condition, age, psychological, educational, and socio-economic conditions, producing measurable goals and objectives in partnership with the family and client.
  • Integrate contributions of the client/child and family into the multidisciplinary care plan.
  • Provide education to clients and families regarding healthy lifestyle, medications, health conditions, etc. as needed.
  • Evaluate and document the effectiveness of the IHH care plan.
  • Responsible for tracking IHH service plans and risk stratification tool.
  • Responsible to sign off on IHH ICM assessments.
  • Responsible to assist care coordinators in referring to PMICs and coordination of documentation with PMICs.
  • Attend Clinical Review Meetings and staffings for residential clients enrolled in IHH.
  • Provide nursing triage and education to foster group care program and other FRI programs as needed.
  • Provide support to foster group care program medication management protocol, appointment scheduling, and other medical/dental/psychiatric support as needed.
  • Liaison for foster group care clients with psychiatric providers, e.g. Vera French Mental Health Center.
  • Document all services provided.
  • Serve as liaison with community providers by providing expert knowledge of the service system.
  • Provide On-Call telephone consultation on a rotating basis.
  • Facilitate communication between the families and care providers, serving as an advocate for the child and family.
  • Assist the family to obtain necessary knowledge of health care financing entities such as Medicaid, SSI, state waivers, and others as needed.
  • Other duties as assigned.

  Qualifications 

Active Iowa Nursing License required. 1-year experience in working with children and their families. Human services/social services experience is preferred.

Must have current proof of car insurance, able to pass driving background check and have reliable transportation.
