The Scott County Health Department (SCHD) is pleased to offer an evidence-based Community Health Assessment aNd Group Evaluation (CHANGE) Tool assessment and technical assistance in developing or strengthening environmental changes and wellness policies to promote physical activity, healthy eating, and chronic disease prevention.

Following completion of the CHANGE Tool, SCHD staff will meet with your workplace and provide free hands on technical assistance to create a culture of wellness best fit for your workplace. Previous workplace successes include:

• establishing a wellness committee;
• increasing healthy vending machine items and marketing;
• strengthening tobacco-free policies;
• providing private and comfortable lactation rooms; and
• developing a comprehensive wellness policy.
