About Us

We are a Primary Care Medical Clinic that serves adults on Medicare. We will have two clinics in the Quad Cities, one in Moline in the City Line Plaza at 1996 1st Street A and a second clinic in Davenport at 2217 Rockingham Road. Both clinics are slated to open late this Fall. Oak Street Health concentrates on serving the underserved population. We offer transportation to and from appointments for our patients who reside in a five mile radius of our clinics. We partner with local nonprofits and other resources in the community to let them know about the services that we provide and in turn refer our patients in need of services and resources to those businesses. We also have a community room at each clinic that provides activities, a computer lab for Medicare patients and also serves as a heating and cooling center during extreme weather conditions. At Oak Street Health we are rebuilding healthcare as it should be!
